Confirmed by the official Steam listing: the game has multiple islands, and unlocking new ones is part of the progression loop. Boss fights, quest completion, and gear upgrades all factor into how quickly you can move on.

The progression loop

The progression loop is: catch fish, sell fish, buy better gear and weapons, complete quests, defeat bosses. Each major boss unlocks the next island. The exact order of islands is not officially documented in the materials we have, so this wiki does not list them in sequence.
